The pancreas lies behind the stomach. It releases digestive enzymes into the small intestine and hormones, including insulin, into the bloodstream. Pancreatitis means inflammation of this organ, but the course and testing depend on whether the process is acute or chronic.

The classic symptom is acute, persistent pain in the upper abdomen, often centered in the epigastrium and sometimes radiating to the back. Pain may be accompanied by nausea, repeated vomiting, fever, tenderness, rapid pulse, or abdominal swelling. Severe disease can affect the lungs, kidneys, circulation, and other organs.
Symptoms overlap with gallbladder disease, a perforated ulcer, bowel obstruction or ischemia, hepatitis, heart disease, and other emergencies. This is why a lipase number should never be used to triage severe abdominal pain at home.
Chronic pancreatitis may produce recurrent or persistent pain, weight loss, greasy or difficult-to-flush stool, diarrhea, vitamin deficiencies, and diabetes. Some people have less pain as the disease advances. Those patterns need planned gastroenterology evaluation rather than repeated acute enzyme panels.

This is a clinical framework, not a self-diagnosis rule. The degree of enzyme elevation does not reliably grade severity, and other abdominal, kidney, medication-related, or systemic problems can raise lipase. Conversely, delayed presentation, advanced chronic damage, or marked hypertriglyceridemia can complicate enzyme interpretation.[1]
Lipase helps digest fat. When pancreatic acinar cells are injured, serum lipase can rise. Compared with amylase, lipase is generally more specific to pancreatic injury and stays elevated longer, making it the preferred enzyme for most suspected acute pancreatitis evaluations.
A marked elevation with characteristic pain strongly supports acute pancreatitis. Milder elevations are less specific and can occur with impaired kidney clearance, gallbladder or intestinal disease, some medicines, critical illness, and other conditions. The laboratory result must be interpreted against the assay-specific upper limit, timing, symptoms, kidney function, and imaging.

Lipase alone cannot:
Routine daily or repeated lipase testing after acute pancreatitis is established usually does not guide recovery. Clinicians follow pain, oral intake, vital signs, kidney and respiratory status, inflammatory course, and complications instead.
Amylase is produced by the pancreas and salivary glands. It may rise earlier and return toward normal sooner than lipase. Salivary disease, reduced kidney clearance, macroamylasemia, and other conditions can raise it. A normal amylase does not exclude acute pancreatitis when lipase or imaging is diagnostic, and adding amylase routinely to an already informative lipase result often contributes little.[1]

Additional testing is chosen to assess cause, complications, hydration, and organ function—not to create a universal “pancreatic panel.”

A CMP already includes total calcium and selected liver-associated markers. Avoid duplicating individual tests unless the clinical question or laboratory method requires it.

Ultrasound is commonly used early to look for gallstones and bile-duct dilation. It may not visualize the entire pancreas well because bowel gas and body habitus can limit the view.
Contrast-enhanced CT can show pancreatic inflammation, necrosis, fluid collections, and alternative diagnoses. The 2024 ACG guideline advises reserving early CT for an unclear diagnosis or lack of expected improvement rather than ordering it routinely in every obvious case. MRI and MRCP can evaluate the pancreas and bile/pancreatic ducts without ionizing radiation and may be useful in selected patients.[1][2]
Endoscopic ultrasound (EUS) can assess small stones, duct abnormalities, masses, and chronic changes when other studies do not answer the question. Endoscopic retrograde cholangiopancreatography (ERCP) is invasive and is generally used for a therapeutic indication, such as selected bile-duct obstruction or cholangitis, rather than as routine diagnostic imaging.
Gallstones and heavy alcohol use are common causes. Others include high triglycerides, high calcium, medicines, procedures, infection, trauma, anatomic abnormalities, autoimmune pancreatitis, and inherited susceptibility. Sometimes no cause is identified after initial testing.[3]

Chronic pancreatitis is a structural disease. Diagnosis often relies on CT, MRI/MRCP, EUS, and the clinical course. Serum lipase and amylase can be normal because damaged tissue may release fewer enzymes; normal levels do not reassure against chronic disease.
When greasy stool, unexplained weight loss, nutrient deficiency, or maldigestion suggests EPI, Pancreatic Elastase-1 can provide exocrine-function context. It is a stool test, not a blood enzyme. A low value supports EPI, but watery stool can dilute the sample and borderline results require clinical interpretation. AGA guidance considers values below 100 μg/g good evidence of EPI and 100–200 μg/g indeterminate; use the reporting laboratory’s interpretation and specialist guidance.[4]
EPI can be treated with prescribed pancreatic enzyme replacement and nutrition support when diagnosed. Do not use over-the-counter enzymes as a diagnostic trial for acute pain.

Pancreatitis does not mean pancreatic cancer. Chronic pancreatitis is associated with increased long-term cancer risk, but most people with pancreatitis do not have cancer. New jaundice, progressive weight loss, a pancreatic mass, a duct cutoff, or persistent unexplained symptoms may lead a clinician to a cancer-focused workup. Lipase and amylase cannot detect or exclude malignancy, and tumor markers are not substitutes for imaging and tissue evaluation. In emergency evaluation, do not delay care to fast or to order tests online. In a planned outpatient cause assessment, follow the instructions for the complete order. Fasting may be required for triglycerides or other paired studies depending on the clinical question. Tell the clinician about alcohol, supplements, prescription and nonprescription medicines, recent procedures, gallstones, kidney disease, pregnancy, and prior attacks.
Severe or persistent abdominal pain, repeated vomiting, jaundice, fever, fainting, breathing difficulty, or dehydration requires clinical triage. A direct-access lipase result cannot provide intravenous treatment, determine organ failure, visualize a bile duct, or rule out another abdominal emergency. Even when an outpatient result is abnormal, the next step may be urgent care rather than adding more blood tests.
