Tests and Strategies That Often Add Limited Value

Repeated antibody concentrations

Repeating TPO antibodies or thyroglobulin antibodies to judge whether treatment is working is generally not useful. Antibody values may remain elevated even when thyroid function is stable, and changes in antibody levels do not tell a clinician how to adjust levothyroxine.

Free T3 for routine hypothyroidism diagnosis

Major Pharmaceuticals initiated a voluntary nationwide recall of selected levothyroxine sodium tablet lots in July 2026 after affected products were found to be subpotent. The recall does not apply to every levothyroxine product. The American Thyroid Association published an alert on July 30, 2026.[8] A state board of pharmacy also posted the manufacturer recall notice.[9]

People taking levothyroxine should not stop treatment, double a dose, or assume their product is affected. Check the manufacturer and lot with the dispensing pharmacy. If an affected product is replaced—or if symptoms return or TSH changes unexpectedly—ask the prescribing clinician whether and when TSH and Free T4 should be reassessed.
