Clinical Significance
Drug Monitoring, Amphetamines, with D/L, Quantitative, Urine - Methamphetamine has two isomers, d-methamphetamine (dextromethamphetamine) and l-methamphetamine (levomethamphetamine). d-methamphetamine is most often illicitly manufactured and is considered a drug of abuse (Crystal meth, etc.). It is approved as a treatment for ADHD and obesity, although its use is very limited. l-methamphetamine is used in certain nonprescription inhalers as a decongestant. This test is a definitive assay using a gas chromatography mass spectroscopy (GC-MS) method of analysis. In routine confirmation testing of amphetamines, the specific methamphetamine isomers are not differentiated, and reported as "Methamphetamine". In situations where this is an unexpected result, it may be useful to determine the relative percentages of each isomer present. The presence of the l-isomer at greater than 80% is consistent with the use of non-prescription inhalers. The presence of the d-isomer greater than 20% is consistent with the use of prescribed medication or illegally manufactured methamphetamine. Quantitative values cannot be used to assess the drug dose, because the drug is extensively metabolized and excreted in the urine.
