The following is a list of what is included in the item above. Click the test(s) below to view what biomarkers are measured along with an explanation of what the biomarker is measuring.

Alt

Alanine transaminase (ALT) is an enzyme found in the highest amounts in the liver. Injury to the liver results in release of the substance into the blood.

Also known as: Aspartate Aminotransferase AST, AST, SGOT

AST

AST (aspartate aminotransferase) is an enzyme found in high amounts in liver, heart, and muscle cells. It is also found in lesser amounts in other tissues.

Also known as: Platelet Count EDTA, PLT, PLT Count, Thrombocyte Count

Platelet Count

A platelet count is a test to measure how many platelets you have in your blood. Platelets help the blood clot. They are smaller than red or white blood cells.
*Important Information on Lab Test Processing Times: Ulta Lab Tests is committed to informing you about the processing times for your lab tests processed through a national lab. Please note that the estimated processing time for each test, indicated in business days, is based on data from the past 30 days across the 13 laboratories for each test. These estimates are intended to serve as a guide and are not guarantees. Factors such as laboratory workload, weather conditions, holidays, and the need for additional testing or maintenance can influence actual processing times. We aim to offer estimates to help you plan accordingly. Please understand that these times may vary, and processing times are not guaranteed. Thank you for choosing Ulta Lab Tests for your laboratory needs.

The FIB-4 Index Liver Health Evaluation (Self Calculation) panel contains 3 tests with 3 biomarkers .

Assess your liver health with key biomarkers (AST, ALT, and Platelet Count) to calculate your FIB-4 Index and evaluate potential fibrosis risk.

How to Calculate Your FIB-4 Index Yourself

You can calculate your FIB-4 Index — a simple, non-invasive way to help evaluate liver health — using individual lab test results instead of ordering the full FIB-4 Index Panel with Reflex to Enhanced Liver Fibrosis (ELF) Score.

This Panel includes the following three tests

  1. Aspartate Aminotransferase (AST)

  2. Alanine Aminotransferase (ALT)

  3. Platelet Count

Plus you need to provide Your Age (in years)

Once you have your results, use the following formula:

FIB-4 Index Formula (Written Description)

The FIB-4 Index is calculated by multiplying your age in years by your AST (Aspartate Aminotransferase) result in units per liter (U/L).
That number is then divided by the product of your platelet count (measured in billions per liter, or 10?/L) and the square root of your ALT (Alanine Aminotransferase) result in units per liter (U/L).

In words:

FIB-4 equals your age multiplied by AST, divided by your platelet count multiplied by the square root of ALT.

FIB-4 Score Interpretation
< 1.30 Low likelihood of significant liver fibrosis
1.30 – 2.67 Indeterminate range — discuss with your provider
> 2.67 Suggestive of advanced fibrosis — further testing may be recommended

By ordering the tests individually:

  • You can calculate your FIB-4 Index without automatically triggering the ELF test charge ($858).

  • You maintain full control over your follow-up testing and costs.

  • You can review your results with a licensed provider or Ulta Lab Tests’ physician network for guidance on next steps.
