How do I order a lab test?

Use this website to select and order your test directly through Ulta Lab Tests.

Where do I go to have my have my blood drawn?

Get your blood drawn at one of our 2,500 approved patient service centers nationally.

  1. When ordering your lab tests, after you have selected your tests, use our online Locator to search our network and find a convenient Patient Service Center near you.
  2. Take your Lab Requisition to the selected Patient Service Center to get your specimen collected.
  3. If your lab test or wellness package requires fasting, then fast as directed prior to your blood draw (special instructions will be noted in the Patient Instructions section of your Lab Requisition order).

Where do I review my results?

Review your test results and biomarkers online.

  1. Within 1 to 3 business days, you will receive an email notification when your results are available (occasionally, complex tests may take between 7-10 business days for results).
  2. Your results will be uploaded into your private and secure healthcare dashboard.
  3. Log in and view intuitive charts that display your data, along with explanations of your biomarker that help provide you an understanding of your results.
  4. Easy to read PDF's of your tests results are also available to share with your physician and healthcare provider.

How do I track my progress?

Your personalized health dashboard makes it easy to view diagnostic changes that occur between your blood tests, allowing you to adjust your wellness plan for even better health.

Do I need to see a physician to get a lab order before getting a blood test through Ulta Lab Tests?

No. You do not need to see a physician to get a lab test, through Ulta Lab Tests. We enable you direct access to lab tests online and through our healthcare professional network, using the same labs as healthcare providers and hospitals, which are CLIA certified and regulated by government agencies. We help you save time and money and you get the same lab results that you can share with your doctor or healthcare provider.

Do you accept insurance?

To keep prices low, Ulta Lab Tests does not accept insurance. CHARGES FROM ULTA LAB TESTS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR. This prohibition applies whether the insurance program is private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.) or public (such as Medicare or Medicaid or any other state or federally funded health plan or program).

May I submit a claim for reimbursement under Medicare or Medicaid?

You may not. CHARGES FROM ULTA LAB TESTS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR. This prohibition applies whether the insurance program is public (such as Medicare or Medicaid or any other state or federally funded health plan or program) or private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.).

Why are you cheaper than the lab or my doctor?

We have implemented industry best practices and controls, including state of the art technology that minimizes our company's overhead. The result is superior customer service while providing our patients quick, accurate and secure test results that keep our costs low so that we can pass along those savings to you.

Where will my laboratory tests be performed?

All specimens are analyzed at National CLIA certified laboratories.

I tried to purchase a lab test and received a message that the test is not available in my state, why?

Ulta Lab Tests offers testing services to residents of almost all states except the three where direct access testing is prohibited by law - New York, New Jersey and Rhode Island.

When will I be able to view my lab results?

Lab results, including Wellness Panels are typically available 1-3 business days after a blood draw has been completed. You will be notified via email once your results are available. Occasionally, complex test results can take between 7-10 business days.

What if I receive an abnormal result?

Abnormalities should be considered an early warning, but do not necessarily mean you have an illness or disease. We strongly recommend that you discuss your results with your physician or healthcare provider for evaluation, further testing and diagnosis.

We will provide additional communication to alert you should our national laboratory identify that your results are abnormal with a critical or panic value requiring immediate medical care. We will also provide additional communication to alert you should your lab test results be positive for a communicable disease.

What do I need to do to prepare for my test?

Most of the individual lab tests offered require no preparation. However, many of our Health and Wellness Panels do require fasting. If there are special requirements prior to testing, (i.e. such as fasting for up to 12 hours), you will be notified in the Patient Instructions located on your Lab Test Requisition.

How do I determine what lab test(s) to order?

We have organized our lab tests in groupings ranging from Conditions to Lifestyle and we have provided filters to help you evaluate hundreds of lab tests and panels that may be of benefit to you.

We also recommend that you consult with your physician or Healthcare Provider.

If I order multiple blood tests, will I need to have more than one needle stick?

No. It is common to order multiple lab tests off of one blood draw where there is just one needle stick.

Should I continue to take my medicine before getting my test?

Yes. You should always take medicine that has been prescribed by a physician. Please reference your Patient Instructions on your Lab Test Requisition for any special requirements.

How do I pay for my tests?

After you have selected and ordered your lab tests directly here online, you will be prompted to provide your payment information to complete the order your tests. Your payment information is entered online through an encrypted transmission and processed by Authorize.net™.

We accept all major credit and debit cards: MasterCard, Visa, American Express and Discover and most Health Savings Account (HSA) debit cards, or medical or healthcare debit card from your insurance carrier.

Is there a fee to have my specimen collected?

Yes, there is a fee associated with collecting your specimen. The standard draw fee is $8.95 but may vary depending on where you choose to have your specimen collected. In the process of placing your order, you will choose where your specimen will be collected from a list of nearby patient service centers. Unless otherwise indicated, the draw fee will be $8.95. Any draw fee greater than $8.95 will be shown when you are choosing where to have your specimen collected. This will provide you the option to choose a patient service center based upon convenience or the draw fee. The draw fee will be added to your cart at check out.

Can I pay by personal check instead of by credit card?

No, at this time we only accept credit or debit cards.

Can I be confident that the credit card and personal information that I provide Ulta Lab Tests online is secure?

Yes. Your online credit card transactions that are performed on our website are fully encrypted and secured by Authorize.net™ for your protection. Once your order has been processed, the only information we retain about your credit card is the last four (4) digits of the card's number for later reference purposes.

How will the charge for my lab tests show up on my credit card statement?

The charge for your lab tests will appear on your credit card statement with the name of our company, Ulta Lab Tests.

What is your refund and exchange policy?

You may cancel an order if: (1) you have not had your blood drawn and (2) the order is cancelled within six months of the date it was purchased. If the order is cancelled in the first 60 days from the purchase date, the purchase price will be refunded minus a $10 cancellation fee; otherwise you will receive a credit for the purchase amount, which credit may be applied to any future order placed within the six-month period. If not cancelled within the six-month period, any unused order will lapse and there will be no refund or credit.
