• Practitioners who can apply for and benefit from a doctor’s account include MDs, DOs, NDs, NPs, DCs, FNPs, Pas, DC, DDS, DMD, OD, DPM, DPT, DScPT, DSN, ENT, GP, GYN, LSA, MLA, MP, MS, OB/GYN, etc.
• Due to state laws we cannot provide our service to practices located in HI, NJ, NY or RI.
• Once your enrollment form has been submitted, you will receive an email confirming that you have successfully submitted your information.
• Once your information has been approved, you will receive a second email confirming your enrollment with a personalized link for your Physician Portal. At that time, you will have the ability to view your physician direct pricing and have the ability to order and review lab tests for your patients.
