How do my clients order a lab test?

1. How do my employees order discount lab tests?
   • The employee and their family may select and order their discount lab tests on your co-branded microsite provided by Ulta Lab Tests.
   • The employee can:
     • Research and select their lab tests or wellness package online.
     • Create and login into their confidential Ulta Lab Tests account.
     • Select and purchase their discounted tests online through Ulta Lab Tests’ secured PCI compliant payment system.
     • Print their receipt for the lab tests ordered.
     • Print their lab test requisition after checkout to take to the Patient Service Center (PSC) they selected.
2. How do my employees redeem our company wellness vouchers for their health screening?
   • If the company offers, and if the employee is covered by free wellness screening voucher, the employee may order their lab tests on the employer’s co-branded microsite provided by Ulta Lab Tests.
   • The employee can:
     • Select their wellness screening voucher online.
     • Create or login into their confidential Ulta Lab Tests account.
     • Print their lab test requisition after checkout to take to the patient service center.

Where does the employee go to have their blood drawn (specimen collected)?

Your employee can have their blood drawn at one of 2,100 patient service centers (PSC) located across the United States (with the exception of HI, NJ, NY and RI).

• In the order process, the employee will be prompted to select a PSC to have their blood drawn or specimen collected.
• The employee has the option to change the location of the PSC selected on their online portal at any time after their order has been placed.
• The employee will take their requisition with them to the PSC to have their blood drawn (specimen collected).
• The requisition is required by the lab. It contains the list of the lab tests to be performed and instructions for the employee to follow.
• If the lab test or wellness package requires fasting, then the employee must fast for 12 hours or as directed prior to the blood draw (special instructions will be noted on the employee’s lab requisition).

How do employees track their progress?

Your employees’ personalized health dashboard makes it easy for them to view diagnostic changes that occur between their blood tests, allowing them to adjust their wellness plan for even better health.

What do your employees do with their results?

You can encourage your employees to track their progress, consult with their physician or other healthcare provider, and retest as directed. Ulta Lab Tests recommends that your employee share and review their results with their physician; however, it is entirely up to the employee whether to share their test results.
