The employee can:

  • Research and select their lab tests or wellness package online.
  • Create or log into their confidential Ulta Lab Tests account.
  • Select and purchase their discounted tests online through Ulta Lab Tests’ secured PCI compliant payment system.
  • Print their receipt for the lab tests ordered.
  • Print their lab test requisition after checkout to take to the patient service center(PSC).
  • Have their blood drawn at one of 2,100 patient service centers located across the United States (with the exception of NJ, NY and RI).
  • Your employee can have their blood drawn at one of 2,100 patient service centers (PSC) located across the United States (with the exception of NJ, NY and RI).
  • During the order process, the employee will be prompted to select a PSC to have their blood drawn or specimen collected.
  • The employee has the option to change the location of the PSC selected on their online portal at any time after their order has been placed.
  • The employee will take their requisition with them to the PSC to have their blood drawn (specimen collected).
  • The requisition is required by the lab. It contains the list of the lab tests to be performed and preparation instructions for the employee to follow.
  • If the lab test or wellness package requires fasting, then the employee must fast as directed prior to the blood draw (special instructions will be noted on the employee’s lab requisition).
  • Your employees review their test results and biomarkers using Ulta Lab Tests’ online system.
  • Within a few business days following the collection of their blood (specimen), your employee will receive an email notification that their results are available (occasionally, complex tests may take between 7-10 business days for results).
  • Your employee’s results will be uploaded into their private and secured healthcare dashboard.
  • The employee may review their results and view intuitive charts that display their data, along with explanations of their biomarkers to assist them in understanding their results.
  • Easy to read PDF's of the tests results are also available to print and share with their physician or other healthcare provider.

Your employees’ personalized health dashboard makes it easy for them to view diagnostic changes that occur between their blood tests, allowing them to adjust their wellness plan for even better health.

  • You can encourage your employees to track their progress, consult with their physician or other healthcare provider, and retest as directed.
  • Ulta Lab Tests recommends that your employee share and review their results with their physician; however, it is entirely up to the employee whether they choose to share their test results.

No. Your employees do not need to see a physician to get a lab test through Ulta Lab Tests. Ulta Lab Tests allows your employees direct access to lab tests through our healthcare professional network. Ulta Lab Tests contracted laboratories are CLIA certified and regulated by government agencies. Ulta Lab Tests allows your employees to save time and money and to share their lab results with their doctor or other healthcare provider.

Charges from Ulta Lab Tests may not be submitted for reimbursement from any insurance or other health care benefit program or payor. This prohibition applies whether the insurance program is private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.) or public (such as Medicare or Medicaid or any other state or federally funded health plan or program).

No. Your employees may not submit to Medicare or Medicaid.

Ulta Lab Tests has implemented industry best practices and controls, including state of the art technology that minimizes overhead. The result is superior customer service while providing patients with quick, accurate and secure test results. Additionally, Ulta Lab Tests affiliates with healthcare providers nationally to extend national pricing into their practices so that their patients can benefit with the cost savings of the Ulta Lab Tests program. Unlike a physician that purchases lab tests for their practice's patients, Ulta Lab Tests purchases on high testing volumes for patients across the United States. Ulta Lab Tests is able to procure national high volume pricing. Ulta Lab Tests has negotiated prompt payment with the national laboratories removing excess costs and instances where uncollectible or delay in payments could occur that would inflate the patient's price for lab tests. Securing advance payment from customers enables Ulta Lab Tests to secure exceptional costs from the national laboratories that are passed directly on to Ulta Lab Tests’ customers.

All specimens are analyzed at national CLIA certified laboratories.

Direct to consumer lab testing is prohibited under the laws of the states of NJ, NY and RI.

Lab results, including the wellness screenings are generally available 1-2 business days after a blood draw has been completed. Your employees will be notified via email once their results are available. (Occasionally, complex test results can take between 5-7 business days.)

Abnormalities should be considered an early warning, but do not necessarily mean that your employees have an illness or disease. We strongly recommend that your employees discuss their results with their physician for evaluation, further testing and diagnosis.

Ulta Lab Tests will reach out to your employees to communicate lab tests that are identified by the lab as either “critical” or “panic” level as well as test results that are positive for HIV or STD's as required by law.

Most of the individual lab tests require no preparation. However some health and wellness panels do require fasting. If there are special requirements prior to testing it will be noted in the instructions that accompany the Ulta Lab Tests requisition form.

No. It is common to order multiple lab tests from a single blood draw where there is just one needle stick.

Yes. Your employees should always take their medicine that has been prescribed by a physician, unless the requisition instructions state otherwise.

As your employees select and order their lab tests they will be prompted for their payment information. The information is entered online through an encrypted transmission secured by Authorize.net™ for their protection. Ulta Lab Tests does not store your employee’s credit card information (with the exception of the last four digits of the card number for reference purposes). Ulta Lab Tests accepts all major credit and debit cards: MasterCard, Visa, American Express and Discover and most Health Savings Account (HSA) debit cards, or debit cards from your health insurance provider.

No, at this time only credit or debit cards are accepted.

Yes. Online credit card transactions performed on the Ulta Lab Tests website are fully encrypted and secured by Authorize.net™. Once the order has been processed, the only information Ulta Lab Tests retains about the employee’s credit card is the last four (4) digits of the card's number for later reference purposes.

The charge on your employee’s credit card statement for their lab test will appear with the name Ulta Lab Tests.

After an order has been processed, your employee may cancel prior to the blood draw with a refund equal to the price of the test less a $10 cancellation fee, if cancelled within three (3) months from date of order. There is no cancellation refund after three months.

Your employees can go to the Ulta Lab Tests Contact Us page and fill out the appropriate form or they may call Ulta Lab Tests. They will need to provide their name and order number for the test that they would like to cancel.

No, the employer will only receive de-identified or non-specific aggregated data.

No, unless required by law. However, the employee may use share their results with their healthcare professional or whomever they chose to share.

Yes the Ulta Lab Tests system provides discounted lab tests for all family members to purchase tests on the Ulta Lab Tests platform. Eligibility for incremental coverage or wellness screenings ordered by the employer will be determined by each employer’s arrangement with Ulta Lab Tests.

  • From Your Dash board select “My Orders”
  • Select the test for the requisition you wish to print
  • Select “Download PSC Requisition”
  • A “pdf” of the requisition will download
  • You can select to “view the requisition”
  • You can select to “print the requisition”
  • Log into your patient portal
  • Go to the “My Orders” page
  • Click on the order you would like to share
  • Under Order Summary, look for “Results Shared” click the “Change” link
  • From there you can review your sharing options: “Only this order” or “All orders”
    • Note: You will only be able to share your results electronically with the health provider that originated the order. You will always have the option to print a copy of your results and share the printed copy with health professionals of your choice.
