How do I order a lab test?

Your lab test will be ordered directly by your physician and/or you may also order your own lab tests on this website we provide to your physician.

Where do I go to have my blood drawn (specimen collected)?

  1. At one of our 2,500 Patient Service Centers
    • If your physician does not draw blood in his/her office:
      • Your physician will provide you a Patient Service Center (PSC) Requisition that contains the list of the lab tests to be performed and instructions for you to follow in preparation for your test.
      • If your test or wellness package requires fasting, then fast for 12 hours or as directed prior to your draw (special instructions will be noted on your lab requisition order).
    • If you order your lab tests directly online:
      • Use our online Lab Test Center Locator to search our network and find a Patient Service Center (PSC) that is convenient to you.
      • Print your Patient Service Center (PSC) Requisition to take to the PSC. It will contain the list of the lab tests to be performed and instructions for you to follow.
      • If your test or wellness package requires fasting, then fast for 12 hours or as directed prior to your draw (special instructions will be noted on your lab requisition order).
    • You have the option to change the location of the Patient Service Center (PSC) selected on your online portal after your order has been placed.
  2. At your physician's office if the service is offered
    • If your physician places the order for your lab test, he/she may have his/her medical team draw your blood or collect your specimen in his/her office.
    • Many practices do not draw blood or collect specimens in their office and refer their patients to offsite Patient Service Centers. (See above)

Where do I review my results?

Review your test results and biomarkers with your physician using our online system.

  1. Review your results with your Physician
    • Within a few business days your physician will receive your results (occasionally, complex tests may take between 7-10 business days for results).
    • Your physician will review your results and may also choose to review these directly with you.
    • Following your physician's review he/she has the option to release your results into your online patient portal.
  2. Review Your Results Online
    • Following the review of your results and the release by your physician, you will receive an email notification that your results are available.
    • Your results will be uploaded into your private and secure healthcare dashboard.
    • Log in and view intuitive charts that display your data, along with detailed explanations of your biomarkers to assist you in understanding your results.
    • Easy to read PDF's of your tests results are also available to print if you choose.

How do I track my progress?

Your personalized health dashboard makes it easy to view diagnostic changes that occur between blood tests, allowing you and your physician to adjust your wellness plan for even better health. Your practitioner will review your unique biomarkers, and discuss personalized recommendations tailored to your specific needs and goals.

What do I do with my results?

Track your progress; consult with your physician and retest as directed.

Do you accept insurance?

To keep prices low, Ulta Lab Tests does not accept insurance. CHARGES FROM ULTA LAB TESTS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR. This prohibition applies whether the insurance program is private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.) or public (such as Medicare or Medicaid or any other state or federally funded health plan or program).

May I submit a claim for reimbursement under Medicare or Medicaid?

You may not. CHARGES FROM ULTA LAB TESTS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR. This prohibition applies whether the insurance program is public (such as Medicare or Medicaid or any other state or federally funded health plan or program) or private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.).

Why are you cheaper than traditional lab companies?

We have implemented industry best practices and controls, including state of the art technology that minimizes our company's overhead. The result is superior customer service while providing our patients quick, accurate and secure test results that keep costs low so that we can pass along those savings to you.

We have affiliated with your physician to extend our national pricing and platform into their practice so that their patients can benefit with the cost savings of our program.

We purchase on extremely high testing volumes for patients across the United States which allows us to offer discount pricing with our national lab partners. Additionally, we have negotiated prompt payment with the National laboratories and removed all excess costs. Securing advance payment from our customers enables our company to secure exceptional costs from the national laboratories and we pass these savings directly on to our customers.

Where will my laboratory tests be performed?

All specimens are analyzed at CLIA certified laboratories.

I tried to purchase a lab test and received a message that the test is not available in my state, why?

Ulta Lab Tests offers testing services to residents of almost all states except the three where direct access testing is prohibited by law - New York, New Jersey and Rhode Island.

When will I be able to view my lab results?

What if I receive an abnormal result?

Abnormalities should be considered an early warning, but do not necessarily mean you have an illness or disease. Your physician will review your results and develop a treatment plan to manage and treat your illness or disease.

What do I need to do to prepare for my test?

Most of the individual lab tests offered require no preparation. However, many of our health and wellness panels do require fasting. If there are special requirements prior to testing, (i.e. such as fasting for up to 12 hours), you will be notified in the Patient Instructions located on your PSC Lab Test Requisition.

If my physician or I order multiple blood tests, will I need to have more than one needle stick?

No. It is common to order multiple lab tests off of one blood draw; where there is just one needle stick.

Should I continue to take my medicine before getting my test?

Yes. You should always take medicine that has been prescribed by a physician. Please reference your Patient Instructions on your Lab Test Requisition for any exception to this rule or other special requirements.

How do I pay for my tests?

After your physician has selected and ordered your lab tests they will coordinate your payment directly online, or if you are authorized to order directly from your physician's website you will be prompted to provide your payment information to complete the ordering of your tests. Regardless of who places the order your payment information is entered online through an encrypted transmission and processed by Authorize.net™.

We accept all major credit and debit cards: MasterCard, Visa, American Express and Discover and most Health Savings Account (HSA) debit cards, or medical or healthcare debit card from your insurance carrier.

Is there a fee to have my specimen collected?

Yes, there is a fee associated with collecting your specimen. The standard draw fee is $8.95 but may vary depending on where you choose to have your specimen collected. In the process of placing your order, you or your healthcare provider will choose where your specimen will be collected from a list of nearby patient service centers. Unless otherwise indicated, the draw fee will be $8.95. Any draw fee greater than $8.95 will be shown when you are choosing where to have your specimen collected. This will provide you the option to choose a patient service center based upon convenience or the draw fee. The draw fee will be added to your cart at check out.

Can I pay by personal check instead of by credit card?

No, we only accept credit or debit cards.

Can I be confident that the credit card and personal information that I give Ulta Lab Tests online is secure?

Your online credit card transactions that are performed on our website are fully encrypted and secured by Authorize.net™ for your protection. Once your order has been processed, the only information we retain about your credit card is the last four (4) digits of the card's number for order auditing purposes.

How will the charge for my lab tests show up on my credit card statement?

Whether you purchased your lab tests through your physician or online through the physician's website, the charge on your credit card statement for your lab test will appear with the name of our company, Ulta Lab Tests.

What is your refund and exchange policy?

You may cancel an order if: (1) you have not had your blood drawn and (2) the order is cancelled within six months of the date it was purchased. If the order is cancelled in the first 60 days from the purchase date, the purchase price will be refunded minus a $10 cancellation fee; otherwise you will receive a credit for the purchase amount, which credit may be applied to any future order placed within the six-month period. If not cancelled within the six-month period, any unused order will lapse and there will be no refund or credit.
