If movement briefly helps but the sensation returns at rest and repeatedly interrupts sleep, restless legs syndrome may be one possibility.

Ferritin and transferrin saturation (TSAT) are useful in the evaluation of clinically significant restless legs syndrome. They help clinicians consider iron treatment, but neither test can diagnose the condition. The iron thresholds used in restless-legs care also differ from ordinary anemia thresholds.

What symptom pattern suggests restless legs syndrome?

The International Restless Legs Syndrome Study Group's diagnostic criteria describe a recognizable combination:

An urge to move the legs, usually with an unpleasant sensation.

Symptoms that begin or become worse during rest, such as sitting or lying down.

At least temporary relief with movement, such as walking or stretching.

Symptoms that occur mainly in the evening or at night, or are worse then than during the day.

A pattern that is not better explained by another medical or behavioral condition.

People describe the sensations differently: crawling, pulling, tingling, or a deep discomfort that is difficult to name. The relationship to rest, movement, and time of day is often more useful than the precise adjective.

Tell your clinician how often symptoms occur, whether they delay sleep or wake you, and whether they affect travel, evening activities, or daytime functioning. A brief symptom diary can make the pattern easier to assess.

Restless legs follows a characteristic pattern, and other explanations must be considered before diagnosis.

What can mimic restless legs?

A painful muscle cramp, persistent nerve-related burning or numbness, positional discomfort, and generalized medication-related restlessness can resemble parts of the syndrome. They do not necessarily follow the same pattern. The NHS overview notes that leg cramps and peripheral neuropathy can cause similar symptoms.

A blood result cannot resolve these differences by itself. Someone can have low iron and a separate cause of leg discomfort. Someone else can have restless legs without conventional iron-deficiency anemia.

New one-sided swelling, warmth, redness, or significant leg pain should not be assumed to be restless legs. Seek prompt medical assessment, and emergency care if it occurs with chest pain or trouble breathing.

What do the iron tests contribute?

Test

What it measures or contributes

What it cannot establish

Ferritin

Helps assess the body's stored iron.

It is not a direct measurement of brain iron, and inflammation can increase it.

Iron and Total Iron Binding Capacity

Measures circulating iron and binding capacity and reports calculated transferrin saturation.

A single result does not diagnose restless legs or determine treatment in isolation.

Complete Blood Count with Differential and Platelets

Identifies anemia and other blood-cell findings that may need evaluation.

A normal blood count does not rule out restless legs or low iron stores.

C-Reactive Protein (CRP)

May add context when inflammation could be distorting the iron pattern.

It is not an RLS test or a routine requirement for every person with restless legs.

The Ferritin, Iron and Total Iron Binding Capacity Panel combines the iron measurements in the first two rows. A result marked “normal” on a laboratory report can still be relevant to a sleep specialist's treatment discussion.

The 2025 American Academy of Sleep Medicine guideline summarizes the following adult iron-treatment considerations. The guideline explicitly describes these thresholds as consensus recommendations that have not been empirically tested.

Iron finding

How it informs the discussion

Ferritin at or below 75 ng/mL, or TSAT below 20%

The statement supports considering iron supplementation, with route selected clinically.

Ferritin in the 75–100 ng/mL range

The statement specifies intravenous rather than oral iron when iron treatment is indicated.

These values are not diagnostic criteria for anemia, universal targets, or instructions to self-treat. An elevated storage result with low saturation requires assessment rather than automatic supplementation. A result at a boundary needs interpretation with the other measurements and clinical circumstances. Children, pregnancy, and advanced kidney disease require separate considerations.

How should you prepare for iron testing?

Ask for instructions before the appointment. The AASM favors morning collection and avoiding iron-containing supplements and foods for the preceding 24 hours. Confirm how to apply that guidance to your prescribed treatment and meals with the ordering clinician. Ulta's Iron and Total Iron Binding Capacity test also lists morning fasting collection instructions; follow the instructions agreed with the clinician and collection laboratory.

Record recent iron treatment, including infusions, and any illness around the draw. Do not stop essential medications independently. Consistent preparation helps make serial results more comparable.

What happens if results suggest low iron?

The clinician considers both treatment and the reason for the low result. Menstrual loss, blood donation, low intake, and impaired absorption may be relevant. Identifying a contributor helps prevent repeated depletion.

Iron replacement may be part of care, but it is not a promise that every symptom will disappear. Route, duration, follow-up testing, and precautions should be specified. Increasing the dose indefinitely in pursuit of a number can cause harm.

If an oral product causes side effects or results do not respond as expected, report that rather than escalating the dose yourself. If intravenous treatment is considered, the clinician should explain why it fits the overall pattern and how response and adverse effects will be monitored.

What if the iron results do not explain the symptoms?

The evaluation should return to the clinical pattern. Review sleep habits, other sleep disorders, medical conditions, and the medication list. Some medicines may aggravate symptoms, but the decision to change them belongs with the prescriber.

Regular sleep timing, appropriate daytime activity, and brief movement or stretching during symptoms can be useful parts of a plan. Persistent sleep disruption warrants care even if laboratory results fall within the printed reference intervals. A sleep specialist or neurologist may help when symptoms are difficult to classify or treat.

What should follow-up measure?

Follow-up should track the person as well as the laboratory results. Record nights affected, time needed to fall asleep, symptom severity, daytime effects, and treatment side effects. Repeat iron testing should occur on the clinician's schedule and account for the route and timing of replacement.

Do my symptoms fit the full clinical pattern, or is a mimic more likely?

How do the storage and saturation results fit together?

Are we using an RLS treatment threshold or assessing ordinary iron deficiency?

What would count as meaningful improvement?

When should we repeat testing or reconsider treatment?

Common questions

Does a low ferritin result prove I have restless legs?

No. Low reserves deserve attention, but diagnosis still requires the characteristic symptoms and assessment for other explanations.

Can I have restless legs with normal hemoglobin?

Yes. The condition is not defined by anemia. A normal blood count therefore does not end the sleep-related assessment.

For a fuller explanation of the measurements, see the iron-studies explainer.
