Autoimmune gastritis is one possible cause: immune damage to the stomach lining can interfere with absorption of both B12 and iron.

The useful question is: Do your symptoms and results suggest a deficiency, an absorption problem, or another explanation? A B12 measurement may be sufficient to support a deficiency diagnosis in some situations. Other people need clarification with methylmalonic acid (MMA), a complete blood count (CBC), iron studies, or tests for an autoimmune cause. Everyone does not need every test. [1]

Ulta Lab Tests can help eligible patients access relevant laboratory information to discuss with a healthcare professional. This article focuses on B12 deficiency and the investigation of autoimmune malabsorption.

How are B12 deficiency, autoimmune gastritis, and pernicious anemia different?

B12 deficiency describes inadequate vitamin availability; autoimmune gastritis describes a stomach disease that can cause it. They are related, but they are not interchangeable diagnoses.

Vitamin B12, also called cobalamin, supports DNA production, red blood cells, and nerves. Deficiency may result from limited intake, medication effects, intestinal disease, stomach or ileal surgery, or impaired absorption. Nitrous oxide exposure can inactivate B12 and create a functional deficiency even when the measured concentration appears reassuring.

In autoimmune gastritis, immune damage affects stomach cells that produce acid and intrinsic factor, a protein needed for normal B12 absorption. Reduced stomach acid can impair iron absorption; reduced intrinsic factor can impair B12 absorption. Iron depletion may become apparent before the classic B12-related anemia pattern. [2]

Pernicious anemia describes the established B12-malabsorption and anemia manifestation associated with intrinsic factor failure. Autoimmune gastritis can exist before anemia develops. NICE therefore uses autoimmune gastritis for the underlying condition rather than treating the terms as identical.

Iron depletion may precede obvious B12 deficiency in autoimmune gastritis; other causes of low iron still need consideration.

When should you consider evaluation for vitamin B12 deficiency?

Testing is most useful when symptoms, a recognized risk, or an earlier result creates a specific question. Fatigue and tingling have many possible causes. It is associated with increased gastric neuroendocrine tumor risk and possible increased gastric adenocarcinoma risk. New or worsening upper-abdominal symptoms, vomiting, early fullness, unexplained weight loss, or gastrointestinal bleeding deserve medical assessment. Endoscopy and surveillance decisions are individualized; antibody positivity does not define one schedule for everyone. [1]

Digestive Health Lab Tests explains other gastrointestinal questions. Inflammation and Autoimmune Blood Tests provides broader context without implying that a large autoimmune panel is needed.

Which situations change the testing plan?

Metformin and neuropathy

Someone taking metformin may have diabetic neuropathy, B12 deficiency, both, or another cause of tingling. B12 evaluation can address a potentially correctable contributor; diabetes tests answer a separate question. Diabetes and Prediabetes Blood Tests explains that distinction.

Pregnancy and breastfeeding

NICE recommends active B12 as the initial test during pregnancy. Availability and interpretation require coordination with prenatal care. Pregnancy-related anemia, iron needs, and symptoms should not be managed through self-selected panels alone.

Nitrous oxide exposure

Nitrous oxide can inactivate B12 despite an apparently normal serum result. NICE recommends MMA or homocysteine as the initial biochemical test when this is the suspected cause. Neurologic symptoms in this setting warrant prompt clinical assessment.

Diet, surgery, and other autoimmune conditions

A vegan diet does not automatically explain deficiency; absorption disorders may coexist. Gastric or ileal surgery changes risk and long-term care. Thyroid disease can independently cause fatigue and cognitive symptoms, so targeted thyroid testing may be appropriate. Persistent neurologic concerns may need the broader assessment described in Brain and Neurological Blood Tests.

What do common combinations of results suggest?

Patterns guide questions; they do not provide a diagnosis without clinical assessment. Numerical examples below are fictional, use pg/mL for B12, and refer to the NICE framework.

Example

Useful next question

B12 145, macrocytic anemia, and numbness

Is prompt treatment and evaluation of the cause needed?

B12 260, normal hemoglobin, and neuropathy

Would MMA clarify deficiency, considering kidney function and competing causes?

B12 220, low ferritin, and normal MCV

Could mixed deficiencies be hiding an expected cell-size change?

Low B12 and positive intrinsic factor antibody

Does the overall pattern support autoimmune malabsorption?

Low B12, negative intrinsic factor antibody, thyroid autoimmunity, and unexplained low ferritin

Does suspicion remain high enough for further gastric evaluation?

Elevated MMA and reduced eGFR

How much could impaired kidney clearance explain?

Apparently normal B12, nitrous oxide exposure, and neurologic symptoms

Could functional inactivation explain the symptoms?

Metformin use, tingling, and borderline B12

Could B12 deficiency coexist with diabetic neuropathy?

High B12 after injections and improving symptoms

What clinical follow-up is needed for the original cause and remaining symptoms?

These are educational examples, not diagnostic rules. Numerical B12 examples use pg/mL and require method-specific interpretation.

How should you prepare for testing?

Follow the instructions for the exact test and complete order. Fasting requirements vary; an iron study, gastrin test, or combined order may have different preparation from a CBC.

Report B12 tablets, injections, multivitamins, energy products, biotin, prescription medicines, and dates of recent treatment. Supplements can change results. When medically safe, diagnostic samples are most informative before replacement begins, but do not interrupt prescribed care or delay urgent treatment to obtain a baseline.

Bring the required identification and requisition or order information, and confirm the collection location accepts the order. Keep previous reports available. Record symptoms and treatment timing so the clinician can interpret changes meaningfully.

What should follow-up measure?

Follow-up should assess recovery and the underlying cause. Review sensation, walking, balance, cognition, fatigue, and any blood-count or iron abnormalities. Persistent symptoms may require reassessment for another explanation.

NICE recommends an initial review around three months after replacement begins, sooner for severe symptoms, and around one month during pregnancy or breastfeeding. This is a clinical review schedule, not a requirement to repeat every laboratory test.

For people receiving intramuscular B12, NICE advises against repeating the initial diagnostic B12 test for routine monitoring. An elevated result after injections does not show that an absorption disorder has resolved. Autoimmune malabsorption may require lifelong replacement; route and dose are clinician decisions. Recovery can take time, and some neurologic effects may persist. HSA and FSA cards are accepted for eligible services; confirm your plan’s requirements. Patients can use results to support a more informed healthcare visit. Confirm the selected test, collection arrangements, preparation, and follow-up plan before ordering.

A focused individual test may be sufficient; a B12 and folate panel or iron-study panel may fit when its components answer related questions. A CBC measures blood-cell findings, and B12-related nerve symptoms can develop without anemia or enlarged red cells. Mixed iron and B12 deficiencies can also produce a normal average MCV. Persistent numbness, balance problems, or other neurologic symptoms require clinical assessment even when the CBC appears reassuring.

Does a borderline B12 result mean I have a deficiency?

Not necessarily. A borderline result leaves uncertainty that depends on the laboratory method, symptoms, risks, and recent supplementation. MMA may help when the additional information would change care. The clinician should also consider kidney function and other possible explanations rather than making a diagnosis from the B12 number alone.

Does high MMA always mean B12 deficiency?

No. Reduced kidney function can elevate MMA independently of B12 deficiency. Interpret the value using the laboratory’s range, creatinine or eGFR, symptoms, and B12 status. An elevated result can add useful evidence, but it cannot identify the cause of every symptom or establish an absorption disorder by itself.

Can autoimmune gastritis cause low iron as well as low B12?

Yes. Autoimmune damage can reduce stomach acid and intrinsic factor, affecting nutrient absorption through different mechanisms. Iron depletion may precede obvious B12 deficiency. However, low ferritin has other explanations, including blood loss and inadequate intake, so it should not automatically be attributed to autoimmune gastritis without further evaluation.

Does a negative intrinsic factor antibody rule out autoimmune gastritis?

No. A negative result does not exclude the condition. If unexplained deficiency and the clinical history keep suspicion high, a clinician may consider additional antibodies, gastrin, or gastroenterology assessment. Endoscopy and gastric biopsies may be needed to investigate stomach-lining changes; repeating nutrient tests alone does not answer that question.

Should I stop B12 supplements before a blood test?

Do not stop prescribed treatment on your own. Tell the clinician and laboratory what you take and when you last took it. Baseline sampling before replacement can be useful when safe, but significant neurologic symptoms need prompt care. Test preparation should follow individual instructions and should not delay necessary treatment.

Do I need every test listed in this article?

No. The list explains available diagnostic questions, not a universal panel. Initial testing, functional clarification, iron or folate evaluation, and investigation of autoimmune causes serve different purposes. Your symptoms, previous results, medicines, and medical history determine which tests could add useful information and which would be unnecessary.

Can I arrange B12 testing online through Ulta Lab Tests?

Eligible patients can explore available testing through Ulta Lab Tests and follow the service’s ordering and authorization process. Direct access does not mean professional review is unnecessary. Confirm preparation and collection requirements, and plan who will interpret abnormal results, investigate their cause, and arrange treatment or specialist care when appropriate.

Your next step with vitamin B12 deficiency testing

Vitamin B12 deficiency deserves evaluation that connects the result with symptoms and the likely cause.
