In its chronic hepatitis B guidance, the World Health Organization uses an APRI above 0.5 as evidence supporting significant fibrosis and above 1.0 as supporting cirrhosis in a defined hepatitis B and resource-limited setting. Those values should not be applied as universal cutoffs for every liver disorder. Transient elastography or another secondary test may be preferable when available.<sup><a href=\"#ref-7\">7<\/a><\/sup><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Clinical perspective:<\/strong> FIB-4 and APRI are triage tools. They help identify who may benefit from additional testing; they do not establish the cause of liver injury or replace imaging, elastography, specialist evaluation, or biopsy when those are needed. Mineral, Bone, and Electrolyte Ratios<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Minerals interact, but current guidance generally favors interpreting the individual values and the broader clinical setting rather than applying a universal ratio cutoff.<\/p>\n\n\n\n<h4 id=\"h-calcium-phosphate-product\" class=\"wp-block-heading\">Calcium-phosphate product<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><code>Calcium-phosphate product = Calcium \u00d7 Phosphate<\/code><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This product was historically used in advanced chronic kidney disease and dialysis care. Current kidney mineral-and-bone guidance emphasizes serial calcium, phosphate, and parathyroid hormone results considered together, rather than treating one universal calcium-phosphate product as an isolated target.<sup><a href=\"#ref-14\">14<\/a><\/sup><\/p>\n\n\n\n<h4 id=\"h-active-to-precursor-vitamin-d-ratio\" class=\"wp-block-heading\">Active-to-precursor vitamin D ratio<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Dividing 1,25-dihydroxyvitamin D by 25-hydroxyvitamin D has been studied in granulomatous disease, lymphoma, and kidney disorders. It is not the standard test for vitamin D status. The NIH Office of Dietary Supplements identifies serum 25-hydroxyvitamin D as the main indicator of vitamin D status; 1,25-dihydroxyvitamin D is tightly regulated and may remain normal until deficiency is severe.<sup><a href=\"#ref-15\">15<\/a><\/sup><\/p>\n\n\n\n<h4 id=\"h-sodium-to-potassium-ratio\" class=\"wp-block-heading\">Sodium-to-potassium ratio<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">A serum sodium-to-potassium ratio does not diagnose adrenal insufficiency or aldosterone excess. Abnormal sodium and potassium values warrant evaluation of the individual results, medications, fluid status, kidney function, glucose, acid-base status, and endocrine context. When primary aldosteronism is suspected, ARR\u2014not serum sodium divided by potassium\u2014is the validated ratio.<\/p>\n\n\n\n<h3 id=\"h-7-iron-handling-and-trace-element-ratios\" class=\"wp-block-heading\">7. Iron-Handling and Trace-Element Ratios<\/h3>\n\n\n\n<h4 id=\"h-transferrin-saturation\" class=\"wp-block-heading\">Transferrin saturation<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\"><code>Transferrin saturation = (Serum iron \u00f7 TIBC) \u00d7 100<\/code><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Transferrin saturation, or TSAT, estimates how much of the blood\u2019s iron-transport capacity is occupied.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A low TSAT may indicate reduced circulating iron availability, but ferritin, CBC results, inflammation, kidney disease, and blood-loss history are also needed.<\/li>\n\n\n\n<li>A TSAT of 45% or higher is a commonly used trigger for further evaluation of possible iron overload, especially when ferritin is also elevated.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Liver guidance recommends beginning suspected iron-overload evaluation with TSAT and ferritin and uses a TSAT threshold of at least 45% to guide further assessment.<sup><a href=\"#ref-16\">16<\/a><\/sup> The <a href=\"https:\/\/www.ultalabtests.com\/test\/ferritin-iron-and-total-iron-binding-capacity-panel\">Ferritin, Iron and Total Iron Binding Capacity Panel<\/a> reports ferritin, iron, TIBC, and percent saturation.<\/p>\n\n\n\n<h4 id=\"h-copper-to-zinc-ratio\" class=\"wp-block-heading\">Copper-to-zinc ratio<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">A copper-to-zinc ratio has been associated with inflammation, nutritional status, and outcomes in observational research. It is not a standardized diagnostic test. Serum copper can rise with inflammation or estrogen exposure, while zinc can fall during acute illness. Supplementing zinc solely to change this ratio could obscure the underlying reason for the result.<\/p>\n\n\n\n<h4 id=\"h-calculated-non-ceruloplasmin-copper\" class=\"wp-block-heading\">Calculated non-ceruloplasmin copper<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Formulas that subtract ceruloplasmin-bound copper from total copper are sometimes used in Wilson disease evaluation. It requires prostate volume from ultrasound or MRI and is therefore not a blood-only calculation. It can be helpful in urologic decision-making, particularly when MRI findings are uncertain.<\/p>\n\n\n\n<h4 id=\"h-psa-velocity-and-doubling-time\" class=\"wp-block-heading\">PSA velocity and doubling time<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">PSA velocity describes the rate of change per year, while PSA doubling time estimates how quickly the PSA value doubles. Both require several reliable measurements over time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Urology guidance states that PSA velocity should not be used as the sole reason for secondary biomarker testing, imaging, or biopsy.
