The panel describes the concentration of cholesterol and triglycerides in a blood sample. It does not directly measure plaque inside an artery or determine a person\u2019s complete cardiovascular risk.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A standard lipid panel generally includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Total cholesterol:<\/strong> cholesterol carried across several lipoprotein categories.<\/li>\n\n\n\n<li><strong>LDL-C:<\/strong> cholesterol carried in LDL-related particles. The value is commonly estimated with an equation.<\/li>\n\n\n\n<li><strong>HDL-C:<\/strong> cholesterol carried in HDL particles.<\/li>\n\n\n\n<li><strong>Triglycerides:<\/strong> the concentration of circulating triglycerides, a major form of stored energy.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">From those results, a laboratory or clinician can calculate <strong>non-HDL-C<\/strong> by subtracting HDL-C from total cholesterol. Some reports also display ratios or other calculated values. ApoB and Lp(a) require separate orders unless they are included in a specifically named panel.<\/p>\n\n\n\n<h2 id=\"h-cholesterol-triglycerides-and-lipoproteins-are-not-identical\" class=\"wp-block-heading\">Cholesterol, triglycerides, and lipoproteins are not identical<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Cholesterol<\/strong> is a waxy substance used in cell membranes and in the production of steroid hormones and bile acids. <strong>Triglycerides<\/strong> are molecules that store and transport energy. Because these lipids do not dissolve freely in water, they travel through blood inside <strong>lipoproteins<\/strong>: particles made of lipids and proteins.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The name of a blood test tells you which aspect of that transport system it reflects:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>LDL-C and HDL-C describe the <em>cholesterol mass<\/em> carried in selected lipoprotein categories.<\/li>\n\n\n\n<li>Triglycerides describe the concentration of a different lipid.<\/li>\n\n\n\n<li>Non-HDL-C describes the cholesterol mass in all particles other than HDL.<\/li>\n\n\n\n<li>ApoB approximates the <em>number<\/em> of atherogenic particles rather than the cholesterol mass inside them.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">These measurements often move together, but they can disagree. LDL-containing particles can enter the artery wall and contribute to atherosclerosis. In general, a lower LDL-C means less cholesterol is being carried in these particles and available to enter arterial tissue.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">LDL-C remains a central treatment target because extensive evidence shows that lowering atherogenic cholesterol reduces cardiovascular events. Yet the meaning of one result depends on context. A laboratory reference interval is a statistical description of results in a reference population; a <strong>risk threshold<\/strong> helps identify a level associated with greater concern; and a <strong>treatment goal<\/strong> is a clinical target selected for a specific person. These are not the same thing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, a person with established atherosclerotic cardiovascular disease (ASCVD), diabetes, chronic kidney disease, familial hypercholesterolemia, or another high-risk condition may have a lower LDL-C goal than an otherwise lower-risk adult. The 2026 multisociety dyslipidemia guideline uses risk-based LDL-C and non-HDL-C goals rather than one universal target for everyone.<sup><a href=\"#ref-1\">[1]<\/a><\/sup><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An LDL-C result cannot show whether plaque is already present, where it is located, or whether an artery is narrowed. For questions about plaque, see <a href=\"https:\/\/www.ultalabtests.com\/blog\/heart-and-cardiovascular\/can-you-reverse-plaque-buildup-in-the-arteries-what-labs-can-and-cant-tell-you\/\">what blood tests can\u2014and cannot\u2014tell you about plaque buildup<\/a>.<\/p>\n\n\n\n<h2 id=\"h-non-hdl-cholesterol-the-useful-calculation-many-reports-overlook\" class=\"wp-block-heading\">Non-HDL cholesterol: the useful calculation many reports overlook<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Non-HDL cholesterol includes the cholesterol carried in every particle other than HDL.<\/strong> It is calculated from a standard lipid panel:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Non-HDL-C = total cholesterol \u2212 HDL-C<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, if total cholesterol is 210 mg\/dL and HDL-C is 50 mg\/dL, non-HDL-C is 160 mg\/dL. This arithmetic example is not an interpretation or treatment goal.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Non-HDL-C includes cholesterol carried in LDL, very-low-density lipoprotein (VLDL), intermediate-density lipoprotein (IDL), remnant lipoproteins, Lp(a), and other ApoB-containing particles. That broader coverage can be useful when triglyceride-rich particles are present.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In most settings, non-HDL-C is not a separate blood draw or additional order. It is derived from total cholesterol and HDL-C. Like LDL-C, however, it measures cholesterol mass\u2014not the number of particles carrying that cholesterol.<\/p>\n\n\n\n<h2 id=\"h-apob-particle-number-rather-than-cholesterol-mass\" class=\"wp-block-heading\">ApoB: particle number rather than cholesterol mass<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">ApoB is a structural protein found on major atherogenic lipoproteins, including LDL, VLDL remnants, IDL, and Lp(a). Each of these circulating particles carries one ApoB molecule. An <a href=\"https:\/\/www.ultalabtests.com\/test\/apolipoprotein-b\">Apolipoprotein B Test<\/a> therefore approximates the concentration of atherogenic particles that can enter the artery wall.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The comparison is easiest to understand this way:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>LDL-C:<\/strong> how much cholesterol is carried in LDL-related particles.<\/li>\n\n\n\n<li><strong>Non-HDL-C:<\/strong> how much cholesterol is carried in all non-HDL particles.<\/li>\n\n\n\n<li><strong>ApoB:<\/strong> approximately how many atherogenic particles are circulating.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"h-when-can-ldl-c-and-apob-disagree\" class=\"wp-block-heading\">When can LDL-C and ApoB disagree?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Particles do not all carry the same amount of cholesterol. One person may have fewer cholesterol-rich particles, while another has more cholesterol-depleted particles. This is a conceptual example, not an individual result.<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">This mismatch is called <strong>discordance<\/strong>. It is more likely in people with elevated triglycerides, diabetes, insulin resistance, metabolic syndrome, obesity, or very low LDL-C achieved with treatment. When ApoB and LDL-C are discordant, evidence suggests risk often tracks more closely with ApoB or non-HDL-C than with LDL-C alone.<sup><a href=\"#ref-3\">[3]<\/a><\/sup><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">ApoB is a targeted risk-refinement test, not an automatic requirement for every person. Current guidance identifies it as particularly useful when triglycerides are above 200 mg\/dL, diabetes is present, or LDL-C has reached a low treated level but residual particle-related risk may remain. A low HDL-C can be associated with insulin resistance, higher triglycerides, tobacco exposure, physical inactivity, and other metabolic risk factors. HDL-C is also used in cardiovascular-risk calculators.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Calling HDL-C \u201cgood cholesterol\u201d is incomplete. HDL particles have diverse functions, and the cholesterol concentration inside them does not capture all of those functions. A high HDL-C value does not neutralize elevated LDL-C, ApoB, or Lp(a), and HDL-C should not be interpreted on its own.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">HDL-C is also not a stand-alone treatment target. Clinical trials of medications that raised HDL-C have not shown that increasing the number itself consistently lowers cardiovascular events when added to contemporary treatment. Lifestyle measures such as regular physical activity, a heart-supportive dietary pattern, avoiding tobacco, adequate sleep, and weight management when appropriate remain valuable because they improve overall cardiovascular and metabolic health\u2014not simply because they may change HDL-C.<sup><a href=\"#ref-6\">[6]<\/a><\/sup><sup><a href=\"#ref-8\">[8]<\/a><\/sup><\/p>\n\n\n\n<h2 id=\"h-triglycerides-and-remnant-related-risk\" class=\"wp-block-heading\">Triglycerides and remnant-related risk<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Triglycerides are a major form of stored energy. After a meal, triglyceride-rich lipoproteins carry dietary fat through the bloodstream; the liver also produces triglyceride-rich VLDL particles. As these particles are processed, remnant particles remain.
