Licensed Physicians and healthcare practitioners may set up a Physician account with Ulta Lab Tests.
  • Qualified practitioners begin the account set-up process by registering via the Ulta Lab Tests website.
  • Once your registration is approved, you will be granted access to your secure online portal, a co-branded microsite, the laboratory test menu and the power of our integrated platform.
The Ulta Lab Tests business model has allowed us to negotiate very competitive pricing with our national laboratory providers.
  • We have implemented industry best practices and controls, including state of the art technology that minimizes our company's overhead. The result is superior customer service, and quick, accurate and secure test results at a low cost.
  • We have affiliated with Physician practices like yours to extend our national pricing and platform into their practice so that their patients can benefit from the cost savings of our program.
  • We purchase on extremely high testing volumes, which allows us to offer discount pricing with our national lab partners. Additionally, we have negotiated prompt payment with the laboratories and removed excess costs. Advance payment from your patients enables our company to secure exceptional discounts from the laboratories and we pass these savings directly on to your patients.
All specimens are analyzed at CLIA certified laboratories.
  • The Physician’s practice has the option to draw the specimen on site.
  • Ulta Lab Tests provides access to a nationwide network of approximately 2,500 draw centers. The Patient Service Centers within the closest proximity to the practice and the patient’s home or work are automatically listed when the tests are ordered through our system.
  • Ulta Lab Tests has put together clinically relevant panels to simplify the ordering process. Additionally, practitioners can fully customize their orders using individual markers or panels, including saving their own unique panels for regular ordering.
  • Ulta Lab Tests’ proprietary platform gives you immediate access and pricing to thousands of tests and markers.

All laboratory orders are placed, tracked, and received via our integrated platform. All results are received instantly upon availability using the same software interface with Ulta Lab Tests and our partnering labs.

If your office does not collect the specimen and refers the patient to a PSC to have their specimen collected, the patient does not require an appointment for their services. However, it may save time to call the PSC to make an appointment or inquire about the lab’s schedule and availabilities.
  • Ulta Lab Tests’ software is set up to for the patient to pay for the tests at the time they are ordered.
  • Your Physician’s practice is not charged for the lab tests.
  • When your practice is placing the order for the lab tests on behalf of their patient, you will collect the patient’s credit card payment information and enter it into the Ulta Lab Tests platform to complete the online order.
  • The patient’s payment information is entered online through an encrypted transmission secured by Authorize.net™ for their protection. Your patient’s payment information is not stored on the ordering system.
  • We accept all major credit and debit cards: MasterCard, Visa, American Express and Discover and most Health Savings Account (HSA) debit cards, or medical or healthcare debit card from your patient’s insurance carrier.
The patient’s online credit card transactions that are performed on our website are fully encrypted and secured by Authorize.net™ for their protection. Once the patient’s order has been processed, the only information we retain about your patient’s credit card is the last four (4) digits of the card's number for order auditing purposes.
Whether your patients purchased their lab tests through your practice or you offer the option for them to purchase online through your Microsite, the charge on their credit card statement for their lab test will appear with the name of Ulta Lab Tests.
As part of the order process, the standard laboratory identifiers that are required include the patient’s name, age, sex, address, and phone number. Ulta Lab Tests also requires the patient’s email address so that they can set up their secure online portal to access their results.
  • Results are available and downloaded to the Physician’s portal within seconds of their availability via Ulta Lab Tests’ proprietary bidirectional software.
  • Optional email notifications are available should the practice elect to utilize this function.
  • In your office or online if you wish. Once you review the results you will have the option to release them to the patient via the Ulta Lab Tests platform.
  • Our system provides an individual patient portal for every patient that purchases a lab on the platform. Upon your approval your patient’s results will be uploaded into their private and secure healthcare portal and dashboard.
  • An email will be sent that includes a link for the patient to access their account. The patient will log in and view intuitive charts that display their data, along with explanations of their biomarkers to assist them in understanding their results.
We provide additional communication to alert you should the testing laboratory identify that your patient’s results are abnormal with a critical or panic value requiring immediate medical care.
  • Ulta Lab Tests does not bill insurance directly.
  • However, your patient’s receipt includes the "CPT Codes” for each lab test and the ICD 9 codes (see below) that most insurance companies require for reimbursement
  • Our platform includes a process the enables the Physician to indicate the specific ICD 9 codes that are associated with the lab test order.
  • Your patient may submit their receipt along with their insurance provider's claim form and if approved the amount that was paid may be applied towards their deductible or direct reimbursement based on their benefits plan.
  • Ulta Lab Tests is fully compliant with the Health Insurance Portability and Accountability Act (HIPAA) and its privacy rules issued by the U.S. Department of Health and Human Services. Our privacy notice can be found at the following link: Notice of Privacy Practices
  • Our business partners and associates have been selected based on their quality of service and are required to comply with the requirements of HIPAA.
  • Ulta Lab Tests is fully compliant with the Health Insurance Portability and Accountability Act (HIPAA) and its privacy rules issued by HHS.
