You or your pharmacist Computer Desk orders your lab tests, Blood Syringe get your blood drawn, and then Results receive your results on-line.

  1. Order Your Lab Tests or Wellness Package

    • You select and order your lab tests on this pharmacy website

      • Research and select your lab tests or wellness package online with or without the guidance of your pharmacist or physician.
      • Create or login to your own confidential Ulta Lab Tests account.
      • Select and purchase your tests online through our safe and secure payment system.
      • Receive an automated confirmation email receipt.
      • Print your lab test requisition order after checkout to take to the Patient Service Center (PSC).

    Or

    • Your Pharmacist places your order for the lab tests you have selected

      • You select and share with the pharmacist the lab tests that you would like to be ordered.
      • Your pharmacy staff will place your order and process your payment through our safe and secure payment system.
      • You will receive an automated confirmation email receipt from Ulta Lab Tests along with instructions to create your own confidential Ulta Lab Tests account.
      • Your pharmacist will provide to you your lab test requisition order to take to the Patient Service Center (PSC).
  2. Get Your Blood Drawn at one of our 2,500 Patient Service Centers

    • Select a Patient Service Center to have your blood drawn or specimen collected.
      • If you place your lab test order online, you will be prompted to select a Patient Service Center (PSC) that is convenient to you.
      • If you order your lab tests through the pharmacy, they will provide you choices of Patient Service Centers (PSC) that are convenient to you.
      • You have the option to change the location of the Patient Service Center (PSC) selected on your online portal after your order has been placed.
    • Take your Patient Service Center (PSC) Requisition with you to the PSC to have your blood drawn (specimen collected).
    • The PSC Requisition is required. It contains the list of the lab tests to be performed and instructions for you to follow.
    • If your lab test or wellness package requires fasting, then fast for 12 hours or as directed prior to your draw (special instructions will be noted on your lab requisition order).
  3. Review Your Results Online

    • Within a few business days, you will receive an email notification that your results are available (occasionally, complex tests may take between 7-10 business days for results).
    • Your results will be uploaded into your private and secure healthcare dashboard.
    • Log in and view intuitive charts that display your data, along with detailed explanations of your biomarkers to assist you in understanding your results.
    • Easy to read PDF's of your tests results are also available to print if you choose and available to share with your physician or other healthcare provider.
  4. Track Your Progress

    • Your personalized health dashboard makes it easy to view diagnostic changes that occur between blood tests, allowing you and your physician to adjust your wellness plan for even better health.
  5. Consult Your Healthcare Provider

    • We recommend that you share and review your results with your physician and healthcare providers.
    • It's up to you whether you share your test results with your healthcare provider, your family, or anyone else.

Hints

  • Lab requisitions are valid for 5 months from the date of approval.
  • Please be sure to check your spam or junk folders to ensure that our email address is in your contacts or safe recipients list.
  • If your results are abnormal, we advise you to consult with your physician or medical professional for the appropriate advice and medical treatment plan.

Not Available in Certain States

  • Currently, New York, New Jersey and Rhode Island state laws prohibit direct-access testing. Our lab tests cannot be conducted at lab locations located within these states.
